In order for an individual to receive a level of care from the Department of Elder Affairs' Comprehensive Assessment and Review for Long-Term Care Services (i.e., CARES) Program and be determined medically eligible for Medicaid nursing facility (NF) services or home and community-based services (HCBS), the AHCA MedServ-3008 form (also known as the 3008 form) is required.  In the past, it was required that this form be signed by a Florida licensed physician.  In keeping with federal and state regulations [42 C.F.R. §424.20; Section 464.003, Florida Statutes], it has been determined that the 3008 form may also be signed by a Florida licensed advanced registered nurse practitioner (ARNP).

Effective immediately, Florida licensed ARNP's may now begin to complete and sign the necessary 3008 form in order to assist an individual in applying for Medicaid NF or HCBS services.  The 3008 form and instructions are accessible at http://elderaffairs.state.fl.us/doea/cares_3008ppp.php.
